What is VIP’s High-SMAS Facelift?
1. Limitations of conventional facelift

An aged face results from not only the skin but also subcutaneous fat layer and special layer called SMAS (Superficial Musculoaponeurotic System), which are located between the skin and muscle layer. Therefore, a simple facelift that tightens only the skin flap wouldn’t have a long-lasting effect but would leave a large scar on the cutting line. This is because the incision is sutured without other layers of the entire skin tissue.

MACS-Lift (Minimal Access Cranial Suspension) was a recently introduced facelift method, a procedure of peeling the skin and tightening the SMAS layer with a thick, medical thread. This surgical method may cause unnatural facial expressions, and may not be effective in improving the sagging of the frontal face and jawline; the result is not long lasting (lasts only several months) without peeling the SMAS layer.

Limitations of the General SMAS Lift

As mentioned above, the SMAS layer has to be peeled with skin and subcutaneous tissues for an effective facelift. However, the general technique of SMAS lift only covers the chin and the corners of the mouth – one third of the face. The remaining parts, such as the mid-face areas including the laugh lines, cheeks and zygoma, are not lifted entirely, causing an imbalance in facial proportion.

Benefits of the High-SMAS Lift

The high-SMAS lift was developed to improve the unsatisfying results of the two procedures mentioned above. The skin and SMAS layer are safely peeled up to the upper zygoma for full facelifting by using this method. The high-SMAS lift enables to peel sagged tissues (skin, fat tissues, SMAS layer) separately from facial muscles and put them back to the original position.

It is also effective in removing the nasolabial folds and improving droopiness of the mid-face areas, sagging jawline and jowls without having an unnatural look and tight corners of the mouth.
If micro-fat grafting is additionally combined with the high-SMAS lift to fill up Indian wrinkles, nasolabial folds, corners of the mouth, eyes and around the chin area, the effect of high-SMAS lift will be much more significant and long-lasting.

Long-lasting effect
The effect of high-SMAS facelift will last long as the sagged tissues are peeled from the facial muscles, put back to the original position and fixed to the solid tissues. If micro-fat graft is performed as maintenance treatment from a 1 to 3 year intervals, the effect of high-SMAS lift will be much more significant and last semi-permanently.
Keeps your natural facial expressions

The sagged soft tissues and SMAS layer are properly peeled from the facial muscles and put back to the original position to be lifted. This method will not cause deformity in the facial muscles.

Compared to the high-SMAS lift, the MACS lift peels only the skin and sutures the SMAS layer with thread; it does not separate the facial muscles and tightens the SMAS layer. As a result, facial expressions may look unnatural.

Leaves minimal scarring

The conventional facelift leaves large scars on the front side of the ears, but VIP’s high-SMAS lift improved this point. Incision takes place along the ear cartilage line to make it inconspicuous from the front. By using the hair-follicle spearing technique, hair will grow on the incision line. Generally the incisions will be made on the back side of the ears and hairline. During the first several months of the recovery (at least 6 months), the redness may remain. However, this will be almost invisible after 6 months to 12 months.

Improves the overall droopiness

The droopy skin from the jawline to the upper zygoma (cheekbone) will be evenly lifted and tightened up without looking awkward.

Maintenance of the position and shape of ears

The loose skin tissues and SMAS layer will be separated from the facial expression muscles, and fixed on to the deep temporal fascia to attain rejuvenation. The tissues around the ear areas will be separated from the suture and fixation point to minimize ear deformation. The incision will be made with the avoidance of excessive tension on the skin for less scarring.

The excessive skin tissues can be re-used as an autologous tissue graft

When the patient has enough excess tissues, it can be grafted to the nasolabial folds to improve the sunken ‘smile line’.

Facial aging occurs as a result of aging skin and volume loss (hollow face by decreased facial fat). The excluded facial parts for the facelift include the forehead, temple, nasolabial folds, and around the tip of the chin area. These areas should be contoured with micro-fat grafting to recover thin tissues for an effective recovery.

In order to maintain the results of the facelift procedure, facial fat graft is recommended. It can maximize the recovery of tissue thickness. Although patients have different methods to manage their facelift results, consecutive facial fat grafting from 1 to 3 years can be done to maintain the long lasting results of the facelift procedure.

The surgeon must have a long training period and vast experience in facelift surgery.
Problems such as facial nerve damage can occur when performed by an inexperienced surgeon.
The swelling may subside slowly for patients with thick skin.

Simple lift

The simple lift makes short incisions and can be done in short hours compared to the high-SMAS lift. It is effective in improving minimal sagging and droopy skin. This method is recommended for patients in their early 30s for a slight improvement.

The operation takes a short period of time and has quick recovery.
There will be slight swelling and a small inconspicuous scar.
The procedure is done under general anesthesia, so the patient will not feel any pain during the surgery.

Detaching the SMAS layer is limited, as small incisions are made. The simple lift can be considered a cheek lift, so if the patient has a severely saggy skin, the high-SMAS facelift is highly recommended for an effective result.
